  2.  Complete LS 203 form-we will send it to you
  3.  Wage records-if you have them
  4.  A picture of the passport, the front page and the page with the exit stamp from the war zone
  5.  Letters of Agreement (LOA)
  6.  Contract with the employer
  7.  Medical report- that the injury is related to your work in a war zone
  8.  Own statement-which contains your personal data, where you worked, in which base, in which position, for how long, for which company, how the injury occurred, whether there were any witnesses to the injury, which doctor diagnosed the injury
